Sports medicine involves treating sports injuries which occur when playing indoor or outdoor sports or while exercising.
The hip joint is one of the body’s largest weight-bearing joints and is the point where the thigh bone (femur) and the pelvis (acetabulum) join.
Limb lengthening is a reconstructive procedure where the deformed bone is straightened or missing bone is replaced.

Dr. Randolph Cohen is a board-certified orthopedic surgeon with fellowship training in pediatric orthopedic surgery. He earned his doctorate at the University of Pennsylvania School of Medicine while completing a fellowship in molecular genetics of bone. He completed his orthopedic surgery residency in Philadelphia at the University of Pennsylvania School of Medicine. Participating hospitals included the Hospital of the University of Pennsylvania and the Children’s Hospital of Philadelphia. He was chosen to be chief resident in orthopedic surgery for the University of Pennsylvania residency program. He then completed a year of fellowship in pediatric orthopedic surgery at the Scottish Rite Hospital for Crippled Children in Atlanta, Georgia.

Dr. Cohen is a member of numerous organizations, including the American Academy of Orthopedic Surgeons, the Pediatric Orthopedic Society of North America and the American College of Surgeons. He has authored more than 15 peer-reviewed publications.

Dr. Cohen’s areas of interests include sports injuries and spinal deformities in children and adolescents.

Dr. Stephen Storer is a board-certified orthopedic surgeon with fellowship training in pediatric orthopedic surgery. He earned his medical degree at the Columbia University College of Physicians and Surgeons in New York City. He then completed his orthopedic surgery residency at the Columbia University New York Orthopedic Hospital Program. Participating hospitals included the Children’s Hospital of New York, New York, Presbyterian Hospital and the Springfield Shriners Children’s Hospital. He served as chief resident for the Columbia University Department of Orthopedic Surgery during the last year of his residency training before completing a year of fellowship training in pediatric orthopedic surgery at Children’s Hospital Los Angeles.

Dr. Storer is a member of numerous organizations, including the American Academy of Orthopedic Surgeons, the American Academy of Pediatrics, the American College of Surgeons and Alpha Omega Alpha, and the medical honor society. He is a past recipient of the prestigious O.R.E.F. grant to study bone biology. His areas of interest include sports injuries and surgery of the hip in children and adolescents.

Dr. Jeremy Frank is a board certified pediatric orthopedic surgeon with advanced fellowship training in pediatric and adolescent sports medicine. He graduated summa cum laude and Phi Beta Kappa from the University of Texas at Austin with a Bachelor of Arts. He subsequently earned his medical degree at Northwestern University School of Medicine in Chicago, Illinois, and then completed an orthopedic surgery residency at the University of Miami-Jackson Memorial Hospital in Miami, Florida. After serving as chief resident at the University of Miami, Dr. Frank then completed a one-year advanced fellowship in pediatric and adolescent orthopedic sports medicine at Boston Children’s Hospital as part of Harvard Medical School under the tutelage of Dr. Lyle Micheli and Dr. Mininder Kocher.

Dr. Frank is a member of several prominent organizations, including the Pediatric Orthopedic Society of North America, the Pediatric Research in Sports Medicine Society, the International Society of Hip Arthroscopy, the American Orthopedic Society for Sports Medicine, the American Academy of Orthopedic Surgeons and the Arthroscopy Association of North America. He has authored numerous book chapters, peer-reviewed publications and has conducted regional, national and international presentations. He was the recipient of the best Pediatric Orthopedic Presentation at the 2010 AAOS Annual Meeting and was an invited lecturer at the Chinese Orthopedic Association Meeting in 2016.

He currently serves as a Team Physician for the United States National Wrestling Team and has traveled both nationally and internationally in that capacity. As part of his commitments with [U18] Sports Medicine, he cares for nine different Broward County High Schools athletic departments. He has served as a team and event physician for American Top Team mixed martial arts gym, Babson College, Northeastern University, the Boston Ballet, the Boston Marathon and the Miami-Dade School System.

Dr. Frank is originally from Chicago, IL, and lives with his wife, two children and three dogs in South Florida. His particular areas of interest include pediatric and adolescent sports medicine and ACL surgery, hip arthroscopy, OCD treatment, shoulder labral surgery, dance medicine and pediatric and young adult fracture care.

Dr. Eric Eisner, is a Board-Certified orthopedic surgeon with fellowship training in pediatric orthopedic surgery and hip preservation surgery. After receiving his B.A. from Johns Hopkins University in Baltimore, Dr. Eisner earned his medical degree at the University of California, Davis. He completed his orthopedic surgery residency at the University of Texas Southwestern in Dallas, which included Parkland Memorial Hospital and Texas Scottish Rite Hospital as participating facilities. During the final year of his orthopedic surgery residency, Dr. Eisner completed a three-month rotation on the pediatric orthopedics service at the Norfolk and Norwich University Hospital in Norwich, England. He then served a one-year fellowship in pediatric orthopedic surgery at Rady Children’s Hospital in San Diego, followed by a second fellowship in surgical hip preservation in Bern, Switzerland.

Dr. Eisner is a member of the American Academy of Orthopedic Surgeons and the Pediatric Orthopedic Society of North America. He has received several research grants and authored numerous scientific articles on topics ranging from articular cartilage biology to adolescent shoulder injuries. His work has also been presented at numerous national and international meetings.

Dr. Eisner’s areas of interest include congenital, developmental and traumatic hip injuries in pediatric, adolescent, and young adult patients as well as pediatric trauma care and prevention.

Dr. Matthew Fazekas is a board-certified pediatrician with fellowship training and board certification in pediatric, adolescent and young adult sports medicine. He graduated summa cum laude from the Honors Program at the University of Georgia with a Bachelor of Business Administration. He subsequently earned his medical degree from the Medical College of Georgia, and then completed a pediatric residency at Monroe Carell Jr. Children’s Hospital at Vanderbilt University in Nashville, Tennessee. Dr. Fazekas then completed a sports medicine fellowship through Harvard Medical School in Boston, Massachusetts, at Boston Children’s Hospital.

Dr. Fazekas has served as a consulting and team physician for the Boston Ballet, Northeastern University, and Emmanuel College. He also provided event coverage for the BAA Boston Marathon in addition to other national mass sporting events. He currently serves as a consulting physician for US Figure Skating.

Dr. Fazekas is a member of several professional organizations, including the American Medical Society for Sports Medicine, the American College of Sports Medicine, Pediatric Research in Sports Medicine Society, Alpha Omega Alpha, the Medical Honor Society, and is a fellow member of the American Academy of Pediatrics.

Dr. Fazekas is originally from Atlanta, GA and lives with his wife, son, and daughter in South Florida. His areas of interest include sports injuries, concussion management, and dance medicine.

Dr. Logan Kaleta is originally from Cleveland, Ohio. He is the Neuropsychologist at Holy Cross Hospital in Fort Lauderdale, FL. He earned his doctorate from Nova Southeastern University, and completed his fellowship at Tampa General Hospital. He has been a Credentialed ImPACT Consultant since 2016. He previously held clinical and academic appointments at Emory University School of Medicine from 2015-2018. His clinical interests include concussion management, sports psychology, performance enhancement, and rehabilitation after injury. He serves on several national committees dedicated towards improving athlete development, sports rehabilitation, and the clinical practice of both Rehabilitation and Sports Psychologists.

Gabriel Casanova is completing his 1st year as the Program Manager for U18 Sports Medicine of Joe DiMaggio’s Children’s Hospital. He came to U18 from FIU, where he is the Director of Sports Medicine. Casanova joined the FIU athletic training staff in 2004 after working with Major League Baseball’s Florida Marlins Organization. With over 20 years of experience working from the high school to the professional levels, Casanova was fortunate enough to gain valuable experience working in the outpatient Physical Therapy department and Sports Medicine Outreach program at Baptist hospital as well as various high schools such as Monsignor Pace, Southridge Senior High, and Gulliver Preparatory.

While with the Florida Marlins, he assisted the medical coordinator with rehabilitation of all players within the Marlins organization, both minor and major league baseball players. In addition to assisting the medical coordinator, Casanova was responsible for the medical coverage of the Gulf Coast League Marlins as well as assisting the head athletic trainer of the Florida State League.

In his 15 years at FIU, Casanova’s main responsibilities include being the Director of Sports Medicine in which he was directly responsible for the supervision of every medical component of the sports medicine team Cardiology, Nutrition, Rehabilitation, Orthopedics, Strength & Conditioning, and Mental Health. He also served Head Athletic Trainer for baseball, swimming, and golf as well as an assistant with the football team. Casanova coordinated all rehabilitation protocols in house in FIU’s state-of-the-art training facility with an extensive hydrotherapy room for use by the student-athletes.

Casanova owns both a master’s of science in physical education and a bachelor’s of science in exercise and sports science from FIU. He is a certified intravenous line administrator / phlebotomist, certified kenisio tape practitioner, and CPR instructor as well as a member of the National Athletic Trainer’s Association (NATA). He currently resides in Miramar with his wife, Lisa and their two kids, Madison and Matthew.

Whitney Chambers is the Senior Physical Therapist for U-18 Sports Medicine Program in Coral Springs. She completed her Masters of Science and Doctorate in Physical Therapy at Duke University in Durham, NC. She has over 15 years of practice in physical therapy with 10 years of specialized practice in sports medicine for young athletes. She holds advanced clinical certifications through the APTA in Sports Medicine (SCS) and Orthopedics (OCS). She is a certified instructor though the Cincinnati Sports Medicine Sportsmetrics Program (ACL injury prevention program). She provides educational workshops to dance studios throughout South Florida to help educate dancers and teachers on strategies that will facilitate the development of a strong and healthy future for the young dancers. She has recently co-authored a chapter on the Dancer’s Hip that is part of the First Edition of the Orthopedic Management of the Hip and Pelvis. Her clinical interest is rehabilitation for the female athlete, dance medicine and injury prevention.

Marcos Davy is a physical therapist and certified strength and conditioning specialist who has been with Memorial Healthcare System since 2011. He received his doctorate degree from the University of Miami’s Department of Physical therapy. Additionally, he received a board certification in sports through the APTA in 2014. Marcos practices at the U18 in Coral Springs with a treatment emphasis on injury prevention and enhancing functional movement patterns.

Leonard Gordon Jr. graduated from the University Of Notre Dame with a BA in 2010. While playing football for the Fighting Irish for four years he sustained multiple injuries, which sparked his passion for a career in physical therapy. He went on to complete his Doctorate of Physical Therapy from the University of Saint Augustine for Health Sciences in 2014. He joined the [U18] Sports Medicine team in 2014 upon graduation. In June 2016, he received a board certification in sports medicine through the APTA. As a former student athlete who received physical therapy for multiple injuries, Leonard relates well to his adolescent patients during their recovery which can be a difficult and frustrating period.

Jeff Haffemann graduated from the University of Wisconsin-LA Crosse in 2014 with a degree in Exercise & Sport Science and later received his Doctorate of Physical Therapy from Nova Southeastern University in 2017. He began working as a physical therapist for [U18] in 2018, which he describes as his dream job. Jeff played wide receiver for the UWL Eagles and was a multi-sport athlete in high school. His experience in playing high school and college sports, along with his passion for working with children and adolescents, allows him to relate to athletes and help them through their recovery.
